FUSE (Frequent Users Systems Engagement)

What is the Frequent Utilizer of System Engagement (FUSE) program?

The FUSE program provides long-term, intensive case management to frequent utilizers of emergency services in Larimer County who are involved in the criminal justice system. FUSE case managers work – in conjunction with community partners – to establish long-term housing and wellness plans for program participants.

Are you eligible for the FUSE program?

Currently, ALL referrals for the FUSE program must come through the Northern Colorado’s Continuum of Care Coordinated Entry process. If you are interested in learning more about the FUSE program, please make sure to check with your service provider and ensure you have been assessed for Coordinated Entry.   For a full list of providers, please visit the Northern Colorado Continuum of Care website.

You may also request an assessment through a Resource Navigator at the Murphy Center for Hope.

How to make an appointment with a Resource Navigator?

Please call the Murphy Center at (970) 494-9940 (after 10 am) for more information or visit the Murphy Center at 242 Conifer Street, Fort Collins, Monday – Friday, at 8 am.  Let the Front Desk staff know that you would like to sign up for an “intake”and be assessed for Coordinated Entry.
